About the role
RN supervises patient care, develops care plans, conducts home visits, supervises caregivers, ensures compliance, and coordinates with physicians and families.
Responsibilities
- Conduct patient assessments
- Create and update care plans
- Supervise caregivers and CNAs
- Monitor patient conditions
- Complete Medicaid/DMAS documentation
- Coordinate with physicians and case managers
- Educate families and patients
- Maintain confidential records
- Respond to emergency situations
Requirements
- Active Virginia RN License
- Minimum 2 years experience
- Home health or hospital experience preferred
- Strong communication skills
- Must pass background check
- Basic computer skills
