Registered Nurse RN Admissions Hospice
About the role
Travels to patients' locations to assess their physical and emotional condition for hospice eligibility and communicates program and services available.
Partners with Account Representatives to meet patient and hospital staff needs for new referrals.
Obtains required signatures and completes necessary documentation to comply with regulations for reimbursement.
Evaluates patients' medical needs, current medications, and special equipment requirements.
Notifies Hospice team members for crisis intervention or special services as needed.
Obtains orders and initiates documentation for crisis care if required.
Obtains approval from Hospice and primary care physicians to admit patients to the program.
Updates referral sources on the status of admissions promptly.
Documents and enters patient information into the electronic medical record in a timely manner.
Notifies the patient care team of admissions via electronic mail to ensure continuity of care.
Arranges patient transportation as needed based on circumstances.
Communicates with applicable department(s) and Pharmacist regarding patient equipment and medications.
Responsibilities
- Communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
- Knowledge of electronic medical record.
- Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.
- Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.
- Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.
- Ability to follow standard precaution guidelines for infection control, including when providing patient care.
- Ability to complete AdventHealth Hospice Care CFL Orientation and Training, as necessary.
- Education: Associate's of Nursing [Required], Bachelor's of Nursing [Preferred], Field Of Study N/A.
- Work Experience: 3+ years registered nurse experience in home health and/or acute care nursing experience; including 1-year experience in the hospital [Required].

Pay
$65,582.40 - $108,646.85
Schedule
- Hospitals 10am-630pm Monday-Friday plus 2 weekend days/month.
- Field Based 11:30am-10pm with every other weekend
- Every Weekend 10am-8:30pm - Every Weekend plus 2 variable days/wk.
- Weekend Bonus Paid.
