Shift: Day
Location: Hospitals, homes, facilities in Arapahoe, Douglas, Denver, Elbert, Jefferson and Adams counties

About the Role

Partners with Account Representatives to meet patient and hospital staff needs for new referrals.
Obtains required signatures and complete necessary documentation to comply with regulations for reimbursement.
Evaluates patients' medical needs, current medications, and special equipment requirements.
Notifies Hospice team members for crisis intervention or special services as needed.
Obtains orders and initiates documentation for crisis care if required.
Notifies the patient care team of admissions via electronic mail to ensure continuity of care.

Responsibilities

  • Notifies the patient care team of admissions via electronic mail to ensure continuity of care.
  • Communicates with applicable department(s) and Pharmacist regarding patient equipment and medications.
  • Travels to current hospice patients hospital rooms who have been admitted.

Requirements

  • Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
  • Knowledge of electronic medical record.
  • Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.
  • Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.
  • Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.
  • Ability to follow standard precaution guidelines for infection control, including when providing patient care.
  • Ability to complete AdventHealth Hospice Care CFL Orientation and Training, as necessary.
  • Education: Associate's of Nursing [Required]
  • Bachelor's of Nursing [Preferred]
  • Field of Study: N/A
  • Work Experience: 3+ years registered nurse experience in home health and/or acute care nursing experience; including 1-year experience in the hospital [Required]

Qualifications

  • Registered Nurse (RN) [Required]
  • Basic Life Support - CPR Cert (BLS) [Required]
  • Driver's License (DL) [Required]
  • Cert Hosp & Palliative Care RN (CHPN) - License [Preferred]

Skills

Schedule

Full Time Shift: Monday through Friday. Hours may be 8:30 AM–5:00 PM, 10:00 AM–6:00 PM, or 11:00 AM–7:00 PM
