Essential Duties and Responsibilities

  • Conducts comprehensive assessments and develops individualized, goal-oriented care plans for patients in Acute Care, Swing Bed, and ER settings.

  • Provides skilled nursing interventions including medication administration, IV therapy, wound care, monitoring of acute/chronic conditions, and emergency stabilization.

  • Demonstrates proficiency in responding to emergent and trauma situations, utilizing advanced life support interventions.

  • Adapts nursing practice to meet diverse patient needs, varying acuity levels, and different care settings.

  • Maintains current certifications and actively participates in ongoing clinical competency validation and professional development.

  • Prioritizes patient dignity, safety, and respect in all interactions, ensuring patients and families are active participants in care planning and decision-making.

  • Identifies and addresses patient/family educational needs, providing clear teaching on conditions, treatments, medications, and preventive care.

  • Initiates discharge planning upon admission and ensures smooth transitions of care, coordinating follow-up services as appropriate.

  • Advocates for patients to ensure their preferences, cultural values, and goals of care are honored.

  • Maintains complete, accurate, and timely documentation in the medical record, including nursing notes, care plans, and patient education records.

  • Ensures continuity of care through clear and thorough shift hand-offs, interdisciplinary communication, and participation in care conferences or rounds.

  • Effectively communicates with patients, families, providers, and team members in a professional and compassionate manner.

  • Functioons as an integral member of the interdisciplinary team, fostering collaboration, professionalism, and accountability.

  • Serves as a preceptor and mentor to new employees, modeling best practices and supporting orientation.

  • Monitors stock levels and communicates resource needs proactively to ensure uninterrupted patient care.

  • Assists coworkers when needed and contributes to a positive, supportive team environment.

  • Upholds professional accountability and ethical nursing practice in alignment with organizational standards and regulatory requirements.

  • Demonstrates critical thinking, sound clinical judgment, and effective decision-making in both routine and emergent situations.

  • Participates in quality improvement, policy development, and departmental initiatives to advance patient care and outcomes.

  • Maintains a positive attitude, resilience, and flexibility in balancing the demands of Acute Care, Swing Bed, and ER responsibilities.

Qualifications

  • Current, unencumbered Registered Nurse (RN) license in the State of Wyoming (or compact state eligibility).

  • Current certifications in BLS, ACLS, and PALS required; TNCC strongly preferred.

  • Minimum of one year of acute care or emergency nursing experience preferred; rural or critical access hospital experience desirable.

  • Strong interpersonal, communication, and organizational skills with a commitment to patient-centered care.
