About the role
Provides clinical leadership in an acute inpatient setting, ensuring patient-centered care through evidence-based nursing practices.
Responsibilities
- Conducts comprehensive patient assessments, identifies priorities, and develops individualized care plans in collaboration with patients, families, and healthcare team members.
- Delivers direct patient care within scope of practice, delegates tasks appropriately, and monitors care quality and safety.
- Maintains compliance with organizational policies, safety protocols, and regulatory requirements (TJC, CMS, DPH).
- Evaluates patient outcomes, revises care plans as needed, and documents progress accurately.
- Educates patients and families, assesses learning needs, and promotes health literacy.
- Acts as an advocate and care coordinator, ensuring seamless transitions post-discharge and appropriate referrals.
- Participates in quality improvement activities, audits, and performance evaluations.
- Maintains professional licensure, completes mandatory education, and reflects on practice for continuous improvement.
Qualifications
- Associate degree in Nursing required; Bachelor's preferred.
- Minimum one year of RN experience or equivalent through residency programs or LPN experience.
- Current RN license recognized by NCSBN Compact or state licensure.
- Strong clinical judgment, communication, and teamwork skills.
