About the role
Manage specialized Peritoneal Dialysis (PD) therapies by safely performing manual or automated exchanges, maintaining sterile technique, and assessing catheter sites to deliver life-sustaining care and prevent infection (such as peritonitis).
Deliver expert acute med-surg and post-operative care by conducting rigorous clinical assessments, managing pain and surgical wounds, and closely monitoring fluid and electrolyte status to promote smooth post-surgical recoveries.
Execute medical and nephrology care plans by precisely carrying out physician orders, administering medications, and tracking subtle changes in patient conditions to ensure rapid, high-quality interventions.
Coordinate holistic, multidisciplinary care with nephrologists, surgeons, dietitians, and discharge planners to guide seamless transitions, optimize long-term renal health, and support safe discharge home.
Empower patients and caregivers through hands-on education regarding PD home routines, medication regimens, and post-surgical recovery care to build independence and confidence.
Responsibilities
- Manage specialized Peritoneal Dialysis (PD) therapies by safely performing manual or automated exchanges, maintaining sterile technique, and assessing catheter sites to prevent infection.
- Deliver acute med-surg and post-operative care, including clinical assessments, pain management, wound care, and monitoring fluid/electrolyte status.
- Execute medical and nephrology care plans by carrying out physician orders, administering medications, and tracking patient condition changes.
- Coordinate multidisciplinary care with nephrologists, surgeons, dietitians, and discharge planners for seamless transitions and long-term renal health.
- Educate patients and caregivers on PD home routines, medication regimens, and post-surgical recovery care.
Requirements
- BLS Provider certification obtained within 1 month of hire (American Heart Association or American Red Cross accepted).
- Registered Nurse credential from the Tennessee Board of Nursing obtained prior to hire.
- Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
