Responsibilities

• Manage complex patient recovery journeys by continuously monitoring physiological changes and adapting care plans to ensure safety during the critical transition toward healing.
• Advocate for patient needs by serving as the central coordinator between physicians, therapists, and specialists, ensuring a unified and high-quality approach to treatment.
• Execute clinical orders with precision and foresight, translating medical instructions into compassionate, hands-on care for patients with high-acuity needs.
• Facilitate a confident transition home by assessing discharge readiness and collaborating with the healthcare team to provide patients and families with the tools they need for a successful recovery.

Requirements

• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Diploma from an accredited school/college of nursing OR required professional licensure at time of hire required.

Schedule

Full Time | Days | 6:00am - 6:30pm | Every third weekend and rotating holidays
