Responsibilities

• Provide thoughtful care to residents based on personalized Plan of Care
• Monitor and track changes in the residents' conditions and needs
• Proactively update appropriate contacts regarding the residents' plan of care, health changes, and concerns

Requirements

• Previous experience in nursing or other medical fields
• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem-solving and critical thinking skills
• Ability to thrive in a fast-paced environment
• Must have a current RN license
