Registered Nurse Patient Navigator

Responsibilities

• Guide patients through complex care pathways within emergency and medical transport networks.
• Coordinate services, facilitate access to diagnostics and treatments, and act as a clinical liaison between patients, families, and multidisciplinary teams.
• Perform patient assessments, develop individualized care plans, and support safe care transitions.
• Provide education to patients and families at each stage of care.

Qualifications & Skills

• Registered Nurse (RN) licensure.
• Strong clinical assessment and care planning skills.
• Excellent communication and interpersonal skills for effective collaboration.
• Ability to navigate complex healthcare systems and support patient-centered care.
