Job Description

Responsibilities

  • Assess and record patient status and identify care needs upon admission/point of care entry.
  • Organize assessment health data accurately, completely, and accessibly.
  • Identify immediate care needs and validate diagnoses with patient/family and other health care team members.
  • Document diagnoses in a way that facilitates the determination of expected outcomes and care plan.
  • Initiate problem list at the time of admission and update as needed.
  • Incorporate age-specific care principles into the patient's care plan.
  • Discuss discharge plan with patient/family and health care team members.
  • Identify teaching needs on admission and throughout the hospital stay.
  • Arrange conferences with patient and/or family and other members of the health team when necessary to plan care.
  • Prioritize and organize nursing interventions based on evidence-based practice and established standards of care.
  • Provide individualized, mutually agreed-upon patient and family education that is founded on evidence-based practice and research guidelines.
  • Document all assessments, interventions, and patient responses.
  • Review consistently the patient's health data to keep the Plan of Care updated.
  • Affirm revisions to the plan of care are systematic and ongoing and driven by measurable realistic goals.
  • Document the patient's progress toward identified outcomes.
  • Identify unmet outcomes and provide direction for continuity of care.
  • Reassess nursing actions for effectiveness in achieving desired outcomes.
  • Communicate with patient/family and health care team members regarding patient care and nursing's role in the provision of care.
  • Collaborate with patient/family and healthcare team members regarding the patient's progress/plan of care.
  • Present relevant patient information at multidisciplinary meetings/conferences for feedback from peers.
  • Made appropriate referrals as needed for continuity of care.

Qualifications

  • Bachelor of Science in Nursing (BSN)
  • Current State of Illinois Registered Professional Nurse
  • American Heart Association Basic Life Support (BLS)
  • Strong interpersonal skills
  • Strong analytic abilities
  • Associate Degree Nursing (ADN) acceptable with commitment to obtain a BSN from an accredited program within three years of first day of employment in clinical nurse role.
  • Additional Requirements for the Emergency Room: ACLS, PALS/ENPC & TNCC within one year of hire
  • Additional Requirements for the Pediatric Emergency Room: Pediatric Advanced Life Support TNCC within one year of hire
  • Additional Requirements for Non-Invasive Cardiology: ACLS
  • Preferred: Experience in specific clinical area
  • Additional Preferences for the Emergency Room and the Pediatric Emergency Room: Trauma Nurse Specialist (TNS), Emergency Communications Registered Nurse, Certified Pediatric Nurse, Certified Emergency Nurse
