Role Overview
The Care Team RN in the Cardiology Outpatient Practice, focusing on Valve and Structural Heart Disease Clinics, provides comprehensive nursing care through face-to-face and remote interactions. Responsibilities include pre-visit preparation, direct patient care, patient and family education, and care transitions.
Key Duties
- Coordinate nursing care across practice settings, ensuring effective communication with providers and team members.
- Implement, evaluate, and communicate all phases of patient care within the nursing care model.
- Support professional nursing practice through leadership roles, mentoring, and quality improvement efforts.
- Delegate patient care based on skill level, patient acuity, and available resources.
- Participate in committees or projects and demonstrate excellent communication, mentoring, and critical thinking skills.
Qualifications & Skills
- Graduation from an accredited nursing program and one year of RN experience, with preference for cardiac experience.
- Current RN licensure, BLS certification, and ability to obtain state licensure and specialty certifications if required.
- Strong communication, teamwork, and computer skills, with adaptability to unpredictable situations.
- Leadership qualities, effective mentoring, and critical thinking abilities.
