Registered Nurse - Observation - F/T Nights

About the Role

As a Registered Nurse at Hackensack Meridian Health, you will function as a caregiver for a specific patient population to achieve desired outcomes. This role focuses on expanding knowledge and skills, providing effective direct care as part of the interdisciplinary team for a variety of complex patients. You will direct and oversee care provided by other caregivers through delegation, validation, evaluation, and coordination, while effectively coordinating patient care and unit activities among nursing peers, physicians, and support services.

Responsibilities

• Deliver nursing care according to the most up-to-date evidence-based research, nursing policies, and standards of care to ensure safe and excellent individualized patient care.
• Assessment, Planning, Implementation, and Evaluation:
  • Collect comprehensive data pertinent to the patient's health or situation.
  • Perform age- and disease-specific assessments independently and systematically, focusing on physiologic, psychological, and cognitive status.
  • Recognize clinical and diagnostic changes in patient status.
  • Identify patient/family education needs based on age, disease, and culturally specific learning differences.
  • Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
  • Formulate age- and disease-specific, patient/family-centered goals and outcomes based on assessment data.
  • Develop and coordinate a patient/family-focused, prioritized plan of care encompassing the continuum of care.
• Communication and Coordination:
  • Communicate, coordinate, and implement the plan of care.
  • Provide ongoing communication with patient/family, nursing colleagues, and other healthcare team members regarding patient status, care, and treatment plans.
• Implementation:
  • Provide or coordinate patient/family education as needed and ensure understanding.
  • Perform care according to established protocols in clinical situations, including communication with appropriate persons and documenting events.
  • Deliver nursing care and treatments reflecting patient needs, advocacy, and evidence-based practice while complying with standards.
  • Perform continuous Point-of-Care electronic documentation of assessments, plan-of-care, interventions, and evaluation of rendered care.
  • Administer medication utilizing bar-scanning and documentation technologies to ensure safe delivery.
  • Facilitate multi-disciplinary discharge of patients to ensure safe transitioning and continuum of care.
• Evaluation:
  • Evaluate progress toward attainment of outcomes.
  • Utilize critical thinking skills based on nursing knowledge to achieve desired patient outcomes.
  • Initiate interventions related to changes in patient condition and unexpected responses to care.
  • Identify ethical issues in practice and bring them to the attention of the team.
  • Recognize and respond to opportunities to enhance patient satisfaction.
• Maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
• Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
• Perform physical requirements: lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand a minimum of 6 hours a day.
• Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
• Perform other duties and/or projects as assigned.

Qualifications

• Current and valid New Jersey Nursing license is required.
• BLS/CPR certification by American Heart Association is required.
• BSN in progress and/or willingness to acquire within three (3) years of hire.
• Six (6) or more months of nursing experience OR active participation in Hackensack Meridian Health’s Nurse Residency Program.
• Telemetry certification is a plus.
• Ability to learn quickly and adapt to changing patient needs.

Pay

The starting rate of pay is a reasonable good-faith estimate of the minimum base pay for this role at the time of posting, in accordance with the New Jersey Pay Transparency Act. The posted hourly rate may be stated as an annual salary in the offer, and vice versa, depending on the level and nature of job duties and candidate credentials. The base compensation determined at the time of the offer may differ based on non-discriminatory factors, including labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding. The minimum rate for this role is $46.25 per hour.

Full-time and part-time (20+ hours/week) positions are eligible for a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits. Certain positions may also be eligible for performance-based incentives, bonuses, commissions, or shift differentials for evening, night, or weekend shifts.
