About the role

The Nurse Navigator guides the entire lifecycle of patient care within the department, acting as a liaison between patients, families, and care providers across all settings.

Responsibilities

  • Consult with patients by phone or in person to determine specific needs and referrals to care.
  • Build a patient-family centered relationship.
  • Provide initial admission assessment, screenings, and assess for barriers to care.
  • Report to the care team.
  • Attend and/or review provider appointments; reinforce education and information provided by caregivers.
  • Develop and deliver a Care Plan with treatment summary to patients and/or obtain consents and declinations; reinforce education on treatments accepted or declined.
  • Organize evidence-based care plans and present them to patients for discussion.
  • Follow up as required with patients and providers.
  • Sustain a patient-centered relationship with the patient and family throughout the course of care and into post-care as needed.

Requirements

  • 2 years of experience working directly with patients and providers required.
  • 2 years of experience in the specialty associated with the application preferred.
  • Minimum of Associate Nursing Degree for positions hired after 07/01/2018.
  • Licensed as a Registered Nurse in the state of Colorado or from a valid compact state required.
  • Basic Life Support (BLS) certification required.
  • Department-specific certifications:
    • If Oncology – Oncology Certified Nurse (OCN) preferred.
    • If Obstetrics – Registered Nurse Certified – Inpatient Obstetric Nursing (RNC-OB) required.

Pay

Hourly pay ranges from $40.56 USD to $64.68 USD, based on relevant education and experience.
