Nurse Navigator- Radiation Oncology
Position Summary
Sinai Hospital is seeking a compassionate and experienced Nurse Navigator - Radiation Oncology to provide community-based care navigation for high-utilizer oncology patients. This role partners closely with physicians and a multidisciplinary team—including nursing, social work, and community health workers—to develop and implement individualized care plans that ensure quality outcomes and seamless coordination of services.
Key Responsibilities
Develop and implement individualized, multidisciplinary plans of care in collaboration with physicians and healthcare team members.
Conduct comprehensive nursing assessments to evaluate patient and family strengths, needs, and ability to manage the disease process.
Assist patients in navigating healthcare and community support systems to ensure timely access to care and follow-up services.
Provide in-person support (hospital, Emergency Department, physician offices, patient homes) and telephonic outreach.
Advocate for patients, families, and caregivers to address barriers and improve access to services and programs.
Conduct data collection, assessment, and reporting to support quality improvement and program growth.
Participate in multidisciplinary initiatives to develop programs and resources for high-risk patients.
Support the continued development of the community patient navigation program.
Perform additional duties as assigned.
Qualifications
Active Maryland RN license
AHA BLS Provider certification
Oncology nursing experience
PREFERRED: Radiation Oncology experience
