About the Role

This position is within the Telemetry Med-Surg Surgical Neuro/Ortho department.

Responsibilities

• Successfully challenge EKG I test upon hire, if previously trained, or complete EKG I program within 90 days of hire.
• Successfully interpret life-threatening dysrhythmia and subsequent treatment.
• Demonstrate safe and correct defibrillation.
• Administer anti-dysrhythmic therapy as ordered.
• Demonstrate basic understanding of patient's daily status, including reason for admission, surgeries/procedures performed, pertinent diagnostic data, learning needs of the patient, and discharge plan.
• Successfully complete ACLS training and maintain current status within 90 days of hire.
• Consistently cooperate with rotation of assignments and floating.
• Demonstrate safe and correct care of the patient with internal cardiac devices.
• Demonstrate understanding of patient multi-system complications, evidenced by identification and reporting of abnormal findings to the appropriate physician in a timely manner and implementation of nursing interventions.
• Monitor hemodynamically unstable patients, assess, report, and intervene appropriately.

Assessment

• Perform initial vital signs and admission history, including introduction to patient and family.
• Assess chief complaint and notify physician within 15 minutes of arrival to unit.
• Obtain and document a complete nursing admission assessment, including physiological and psychological factors to develop an interdisciplinary plan of care within eight hours of admission.
• Perform and document an accurate systems assessment at the beginning of each shift.
• Respond to diagnostic data and initiate appropriate action or intervention.
• Assess patient education needs on admission and daily.
• Assess psychosocial needs specific to patient's age, growth, and development.

Planning

• Initiate an interdisciplinary plan of care within eight hours of admission.
• Establish priorities for patient care based on patient needs as evidenced by completion of assigned duties within shift.

Implementation

• Verbalize understanding of patient diagnosis and interdisciplinary plan of care.
• Safely and correctly initiate and document nursing interventions in emergency situations.
• Perform 24-hour chart check at the beginning of each shift.
• Administer, observe, and document intravenous therapy according to hospital policy.
• Administer medications within 30 minutes of scheduled time. Observe and document effectiveness, recognize and report adverse reactions immediately.
• Provide individualized instructions each shift to patient/significant other regarding the plan of care.
• Demonstrate safe and correct use of unit-specific equipment.
• Ensure each patient's general hygiene needs are met daily.
• Perform pre-operative and post-operative care and instruction for each assigned surgical patient during shift.
• Adhere to standard precautions, including hand hygiene guidelines at all times.
• Document specific patient valuables on designated form when received and immediately arrange for appropriate storage.
• Perform dressing changes using aseptic technique.
• Collect and label specimens per policy when ordered by physician.
• Perform bedside glucose monitoring according to policy.
• Administer internal feedings via NG tube, G tube, and/or J tube.
• Report all abnormal findings immediately to physician.
• Initiate wound and skin protocol upon identification of risk.
• Perform ostomy care to include irrigation, appliance usage, and reinforcement of teaching.
• Insert intermittent or indwelling urinary catheter using aseptic technique.
• Administer oxygen therapy according to physician order.
• Transfer patients in/out of the unit efficiently by obtaining a complete and correct set of physician's orders. Complete documentation of patient transfer summary and ensure appropriate completed referral forms accompany intra-facility transfers, including giving/receiving of report.
• Demonstrate safe and correct use of restraints.

Evaluation

• Document/report patient responses or outcomes resulting from: nursing interventions, treatments/procedures, interpersonal interactions, and patient/family education.
• Evaluate and document patient's progress towards established goals/objectives every shift.
• Update/revise patient care problem list and individualized patient care standards based on patient/family responses, progress towards goals, and/or change in medical status every shift.
• Evaluate patient length of stay and related transfer/discharge planning needs based on anticipated problems, internal or community resources, and support systems.

Professional and Staff Development

• Obtain 100% of mandatory hospital and unit-specific in-services annually, prior to evaluation date.
• Plan, present, and document unit in-service as required by level.
• Document competency in clinical skills annually.
• Attend staff meetings as required.
• Use established lines of communication for resolution of problems and interpersonal conflict.

Leadership

• Attend 75% of staff meetings.
• Function as a Charge Nurse, possess clinical knowledge of all patients on the unit, triage and assign beds to promote patient flow.
• Attend a Charge Nurse course.
• Serve as a preceptor/clinical resource.
• Actively participate in unit-based PI.

Requirements

• Current FL Nursing License.
• Current Basic Life Support (BLS) and Current Advanced Cardiac Life Support (ACLS) certifications.
• If in cost center NURSING- 6 DE HMT SOUTH, RN is required to obtain Non-Violent Crisis Intervention Certificate within 90 days of employment.
• Minimum 1 year of RN experience (Level II).
