Role Overview
Key Responsibilities
Provide compassionate, high-quality direct nursing care by implementing and monitoring personalized patient care plans.
Coordinate multidisciplinary care, ensuring seamless collaboration across disciplines.
Assess and plan patient transitions, including discharge planning, to promote safe, successful outcomes.
Document and communicate patient condition changes accurately to support effective team decision-making.
Qualifications & Skills
- Licensure: Current RN license from the Wisconsin Board of Nursing or a valid multi-state license.
- Certification: BLS Provider certification within 30 days of hire, accepted from AHA or ARC.
- Education: Diploma from an accredited nursing school or equivalent professional licensure.
