About the role

Provide direct nursing care to long-term care residents in accordance with established clinical standards.

Responsibilities

• Implement and monitor individualized care plans, documenting and communicating changes in condition.
• Cook/Coordinate with physicians and interdisciplinary teams to carry out orders and ensure seamless care.
• Promote resident safety and support effective discharge planning.

Requirements

• Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse obtained prior to hire date or job transfer date required.
• Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
