More than 100 employees support the agency’s mission to improve health and quality of life by providing culturally competent services to roughly 16,000 patients annually, the majority low‑income. Services include Family Medicine, Internal Medicine, Women’s Health, Urgent Care, Dental, Integrated Behavioral Health, case management, health education and benefits enrollment.

Job Summary

The RN/LPN works collaboratively as an active member of the interprofessional team to provide therapeutic patient‑ and family‑centered care. The role systematically collects and assesses patient data, develops, executes and evaluates care plans, uses evidence‑based practice, coordinates care across settings, considers safety, effectiveness and fiscal accountability, participates in quality improvement initiatives, and pursues professional development.

Essential Functions

• Provide coordinated, patient‑ and family‑centered nursing care by collaborating with the interprofessional team across settings.
• Telephone and MyChart triage: assess symptom severity and direct patients to the appropriate level of care (scheduled visit, same‑day slot, walk‑in/urgent care, or emergency).
• Address medication refill requests by refilling per protocol, forwarding pending prescriptions to the provider team, or advising patients to schedule an appointment.
• Review and triage lab/x‑ray results, notifying physicians or other providers of critical values.
• Empower patients, families and care partners by establishing shared goals, supporting care coordination and using evidence‑based information for informed decision‑making.
• Call patients with abnormal results as directed and report required results to governing bodies (e.g., STI and lead reporting).
• Provide patient education on medication regimens and document in the medical record.
• Seek educational opportunities to increase clinical competence through in‑house materials, conferences and community resources.
• Demonstrate compassion through therapeutic relationships, attentive assessment and respectful engagement throughout the continuum of care.
• Promote a positive work environment that fosters cooperation and collaboration among programs, services and co‑workers.
• Ensure high‑quality care by systematically assessing data, applying evidence‑based practice, prioritizing safety and fiscal responsibility, participating in continuous quality improvement and maintaining professional development.
• Administer oral, subcutaneous, intradermal and intramuscular medications per established protocols.
• Perform nurse visits and document encounters within 72 hours.

Education and Experience

• Graduate of an accredited nursing program.
• Current RN or LPN license in the State of Wisconsin.
• Previous experience with patients of all age groups.
• Experience within a community health center and with EPIC preferred.
• Two years of general nursing in a clinic or hospital setting strongly preferred.
• Vaccination against influenza (proof required prior to orientation) as a mandatory employment condition.
