Responsibilities

• Provide one‑on‑one skilled nursing care and treatment to patients in their home or facility.
• Assist the registered nurse in assessing patients’ conditions and contribute information for care planning and evaluation.
• Demonstrate knowledge of home health and discipline‑specific patient‑care skills required by the care center.
• Perform comprehensive patient assessments as assigned and collaborate with a multidisciplinary team to develop or implement individualized plans of care per physician orders.
• Check vital signs, dress wounds, administer injections and medication, and educate patients and families about medical conditions.
• Provide patient episode management per protocol.
• Complete documentation timely, accurately, and at the point of care according to industry standards.

Qualifications

• Current, unencumbered license to practice as a Licensed Practical Nurse in Massachusetts or eligible compact state licensure.
• Current, unencumbered license to practice as a Registered Nurse in Massachusetts or eligible compact state licensure.
• Current CPR certification.
• Comfortable using technical tools and EMR software for visit documentation.
• Valid driver’s license (required).

Pay

Up to $50.00 per hour.

Schedule

• Full‑time and part‑time positions available.
• Flexible scheduling with weekday, evening, overnight, and weekend shifts.
• Per‑diem and supplemental holiday shifts as needed.
• Standard shifts: day, evening, overnight, weekends.
