About the Role

As a Registered Nurse at Hackensack Meridian Health, you will function as a caregiver for a specific patient population, focusing on expanding your knowledge and skills to provide effective direct care as part of the interdisciplinary team. This role involves directing and overseeing care provided by other caregivers through delegation, validation, evaluation, and coordination, while effectively managing patient care and unit activities among nursing peers, physicians, and support services.

Responsibilities

  • Deliver nursing care according to evidence-based research, nursing policies, and standards to ensure safe and excellent individualized patient care.
  • Assessment:
    • Collect comprehensive data pertinent to the patient's health or situation.
    • Perform age- and disease-specific assessments independently, focusing on physiologic, psychological, and cognitive status.
    • Recognize clinical and diagnostic changes in patient status.
    • Identify patient/family education needs based on age, disease, and culturally specific learning differences.
  • Planning:
    • Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
    • Formulate age- and disease-specific, patient/family-centered goals and outcomes.
    • Coordinate a plan of care that is patient/family-focused, prioritized, and encompasses the continuum of care.
  • Implementation:
    • Communicate, coordinate, and implement the plan of care with the patient, family, and healthcare team.
    • Provide ongoing communication regarding the patient's status, care, and treatment plans.
    • Deliver age- and disease-specific care in an organized and timely manner.
    • Provide or coordinate patient/family education and ensure understanding.
    • Perform care according to established protocols, including communication and documentation.
    • Demonstrate evidence-based nursing practice, advocacy, and compliance with standards.
    • Administer medications using bar-scanning and documentation technologies for safe delivery.
    • Facilitate multi-disciplinary discharge planning for safe patient transitions and continuum of care.
    • Perform continuous Point-of-Care electronic documentation of assessments, plans, interventions, and evaluations.
  • Evaluation:
    • Evaluate progress toward attainment of outcomes.
    • Utilize critical thinking to achieve desired patient outcomes.
    • Initiate interventions for changes in patient condition or unexpected responses to care.
  • Identify and address ethical issues in the practice setting.
  • Enhance patient satisfaction by responding to opportunities for improvement.
  • Maintain required competencies, certifications, and continuing education to meet facility accreditation and licensing requirements.
  • Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
  • Perform physical tasks: lift a minimum of 35 lbs., push/pull a minimum of 10 lbs., and stand a minimum of 6 hours per day.
  • Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
  • Perform other duties and/or projects as assigned.

Requirements

  • A minimum of an Associate in Nursing or diploma in Nursing degree.
  • BSN in progress and/or willingness to acquire within three (3) years of hire (applies to team members hired on or after November 2019).
  • For team members hired after June 1, 2021, BSN is required within five years of hire per union contract (CC456U, CC689U, CC891U). Advanced master's degree in nursing programs satisfy the BSN requirement (e.g., Associates to MSN).
  • Six (6) or more months of relevant nursing experience OR active participation in Hackensack Meridian Health's Nurse Residency Program.
  • Ability to learn quickly, adapt to changing patient needs, and demonstrate accountability for improving patient and caregiver lives.
  • Strong focus on teamwork, dedication to ongoing education, and passion for delivering high-quality care with a patient-centered approach.

Preferred Qualifications

  • National Certification in area of specialty (preferred when eligible).

Licenses and Certifications

  • Current and valid Registered Nurse license to work at the assigned HMH location by date of hire.
  • BLS/CPR certification by the American Heart Association required within 60 days of hire.

Pay

Minimum starting rate of $45.73 per hour. The posted rate is a reasonable good faith estimate of the minimum base pay for this role at the time of posting. The actual base compensation may vary based on non-discriminatory factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding. Some roles may also be eligible for performance-based incentives, bonuses, commissions, or shift differentials for evening, night, or weekend shifts.
