About the Role
Under general supervision of a Nursing Supervisor, performs community health nursing services as a public health nurse to provide preventative medical services.
Responsibilities
- Works clinical nursing programs as assigned, including but not limited to:
- Immunizations, Travel Immunizations, Cancer Screening, Targeted Case Management
- Prenatal-5, Baby Your Baby, Immigration, STD/HIV, Tuberculosis, Viral Hepatitis, Communicable disease
- Provides skilled community health nursing care:
- Assesses individual or family health needs
- Creates nursing care plans and case management using Weber-Morgan Health Department’s standing orders and following state and federal recommendations
- Makes appropriate referrals as necessary
- Provides community health education
- Provides clinical services:
- Administers immunizations to adults and children
- Performs clinical breast exams and pap smears
- Collects and prepares specimens for laboratory testing
- Administers appropriate medication and treatments, including injections and oral medication administration
- Orders additional follow-up and treatments (e.g., diagnostics, imaging, medications)
- Conducts communicable disease investigations as assigned (e.g., interviews, contact tracing, treatment, and follow-up)
- Answers communicable disease questions from the public and other health professionals
- Carries out appropriate public health activities according to specific program contractual requirements
- May conduct home visitation visits per program guidelines to eligible families and individuals
- Evaluates and sets priorities for public health services
- Participates in specified community programs, provides health education, and collaborates with other agencies to meet objectives
- Supports outreach clinical activities as assigned
- Participates in crisis response (e.g., pandemic response, bioterrorism, natural disasters)
- Protects client confidentiality and secures protected health information
- Processes departmental actions while maintaining confidentiality
- Types letters, statements, narrative and statistical reports; records minutes, agendas, and other documents as required
- May draft correspondence as needed
- Performs other duties as assigned
Requirements
- Must be a licensed Registered Nurse in the State of Utah at time of hire
- Must possess a current, valid Utah Driver’s License and operate a motor vehicle safely
- Must complete a pre-employment background check prior to starting the position
Qualifications
- Education: Graduation from an accredited school of nursing with an Associate’s or Bachelor’s Degree
- Experience: None required
Knowledge, Skills, and Abilities
- Knowledge:
- Community health nursing theory and practice
- Medications, immunization schedules, and ACIP guidance
- General communicable disease control, common communicable diseases, Universal Precautions
- Public health education (e.g., nutrition, smoking cessation, exercise guidelines, disease prevention)
- Organization, functions, policies, and procedures of the agency
- New developments in community health nursing
- Skills:
- Basic clinical nursing skills (e.g., medication administration, injections, specimen collection, venipuncture)
- Typing and using computer hardware/software (e.g., Word, Excel, NextGen Enterprise, EHR systems)
- Operating office equipment (e.g., fax, copy machine, cash register, iPads, laptops, radios)
- Using medical equipment (e.g., blood pressure machines, stethoscopes, thermometers, phlebotomy supplies, sterile equipment)
- Abilities:
- Apply community health nursing theory in clinical settings
- Assess patient needs, recognize contraindications, and provide skilled nursing care and referrals
- Create and maintain accurate medical records
- Follow written and verbal instructions, communicate effectively both verbally and in writing
- Establish and maintain effective working relationships with employees, other departments, and the public
- Work independently, efficiently, and productively while maintaining composure, sensitivity, and confidentiality (HIPAA compliance)
- Work under stressful conditions, use logic and reasoning to identify problems and solutions
Physical Demands
- Frequently required to sit, talk, hear, stand, walk, use hands/fingers to handle objects, reach with hands/arms
- Frequently required to climb or balance, stoop, kneel, crouch, crawl, and taste/smell
- Must occasionally lift and/or move more than 25 pounds
- Requires close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus
Work Environment
- Moderately noisy work environment
- May require travel to clients’ homes
- May be exposed to contagious or infectious diseases and require use of personal protective equipment
Career Ladder
- RN I to RN II:
- Satisfy minimum requirements of RN II job description
- Complete a minimum of one (1) year as an RN I or equivalent position with related experience
- Demonstrate proficiency and obtain required credentials to operate at a professional level in clinical nursing services and other approved programs
- RN II to RN III:
- Satisfy minimum requirements of RN III job description
- Complete a minimum of 5 years as an RN II or equivalent position with related experience
- Specialize at an expert level and operate in a lead or advisory capacity for one or more clinical programs
- Complete required and elective leadership trainings provided by Weber County
- Demonstrate leadership qualities, mentor less experienced staff, and present a professional image
- Complete DIS training and Passport to Services Training
- Complete EHR system super user e-learnings
- Demonstrate proficiency in reporting, financials, and proper diagnosis/documentation
- Maintain a positive working attitude and highest level of professionalism, conduct, and productivity
- Complete at least 2 of the following:
- Quality improvement project pertaining to the nursing division (report to staff/Board of Health)
- Complete CIC study and exam
- Prepare and present a case report on a complex communicable disease investigation
- Prepare and present a topic to the Board of Health
