Registered Nurse – Hospice – Full-Time-Days

About the Role
As a Registered Nurse in Hospice, you’ll impact the lives of hospice patients and their families while growing both personally and professionally. This position requires an RN experienced and knowledgeable in hospice care within assisted living and nursing home facilities. You’ll serve as a frontline contact for patients, families, physicians, and hospitals for hospice referrals, coordinate patient services, and communicate with field colleagues. This full-time opportunity is with Lawrence Community Health Services–Jansen Hospice and Palliative Care, located offsite in Scarsdale, NY.
Responsibilities
- Provide hospice care in assisted living and nursing home facilities.
- Apply understanding of the Medicare hospice benefit, hospice diagnoses, and end-of-life disease processes.
- Coordinate patient services and communicate with interdisciplinary team members.
- Act as a liaison between patients, families, physicians, and hospitals for hospice referrals.
Requirements
- Two years of experience in a hospice or home care agency.
- Bachelor’s degree in nursing.
- Current New York State RN license.
- BCLS certification (American Heart Association).
- Valid Driver’s License and reliable car.
- Excellent communication skills and computer proficiency.
Preferred Qualifications
- 3-5 years of hospice experience in assisted living and nursing home facilities.
- Ability to be flexible, apply critical thinking skills, and function independently.
- Strong commitment to working as part of an interdisciplinary hospice team.
Schedule
Full-time, Monday through Friday, 8:30 AM–4:30 PM.
Pay
Salary range: $133,595–$153,220 annually.
