Registered Nurse Homecare Per Diem

About the role

As a Registered Nurse Homecare in Somerset County at VNA Health Group, you’ll be an integral part of a team dedicated to providing high-quality, compassionate care to patients who need it most. You’ll make a direct impact on their lives by assessing their health needs, developing personalized care plans, and empowering patients and families with the knowledge they need to live healthier lives.

Responsibilities

• Start with a two-week clinical orientation to learn everything you need to succeed – from key procedures to our EMR system, safety protocols, and compassionate care.
• Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
• Educate and guide patients and families through their healthcare journey – helping them understand their conditions and the path to better health.
• Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
• Record and track changes in patient conditions, updating care plans to reflect their progress and needs.

Requirements

• Registered Nurse (RN) with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare!).
• Strong verbal and written communication skills – you’ll be guiding and educating patients and families.
• Proficient with technology – navigating EMR systems and using digital tools to enhance patient care.
• A reliable car and valid driver’s license – you’ll be traveling to patient homes, so your own transportation is key.

• Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.
• Direct client and customer contact, requiring your physical presence.

Pay

The compensation range for this position is USD $42.00 to USD $50.00 per hour. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
