Per Diem Home Care Registered Nurse

About the role
Assured Quality Homecare (ASQ) is seeking an experienced Registered Nurse to provide clinical leadership and oversight for private-duty home care clients throughout Rhode Island. This position is designed for a weekday clinical professional. Admissions, assessments, medical appointments, and provider collaboration primarily occur during regular business hours. This is not a night- or weekend-focused per diem role.
As a Per Diem Private Duty RN, you will guide clients through the most important parts of their care journey—from the initial assessment to ongoing clinical oversight.
Responsibilities
- Conduct in-home assessments, admissions, and client intakes
- Develop, review, and update individualized care plans
- Supervise CNAs and provide client-specific instruction
- Collaborate with physicians, families, hospitals, and other providers
- Accompany clients to medical appointments when needed
- Support hospital discharges and safe transitions home
- Provide medication management, wound care, and other skilled nursing services
- Document clinical updates accurately and communicate concerns promptly
Requirements
- Active, unrestricted Rhode Island RN license
- Current CPR certification
- Five or more years of RN experience
- Five or more years of care planning, case management, or patient-navigation experience
- Experience in home care, rehabilitation, long-term care, assisted living, hospice, or a related setting preferred
- Consistent weekday business-hour availability
- Strong assessment, documentation, and communication skills
- Valid driver’s license, reliable vehicle, and current auto insurance
- Ability to travel throughout Providence, Kent, and Washington counties
- Ability to lift up to 50 pounds and assist with client mobility when required

Schedule
Per diem/part-time with consistent Monday-Friday business-hour availability.
