Registered Nurse - Homecare - New Grads

Responsibilities

• Provide and document skilled nursing care tailored to individual patient needs based on physician orders
• Re-evaluate patient needs regularly and utilize the nursing process for care planning, implementation, and evaluation
• Initiate and modify care plans as patient conditions change, ensuring safety and effectiveness
• Administer medications and treatments, monitor outcomes, and document interventions
• Ensure a safe environment, respond to condition changes, and report to supervisors or physicians
• Review care plans, identify education and discharge planning needs, and support patient goals
• Document collaboration with physicians and communicate relevant information
• Adhere to professional standards, legal regulations, and organizational policies
• Participate in team meetings, trainings, and perform assigned duties

Qualifications

• Graduate of an accredited nursing program
• Valid nursing license
• Current CPR certification
• Successful completion of orientation and training program
• Minimum GPA requirement, with consideration for exceptions
• Current resume, clinical hours, and references (preferably professional)
• PPD or chest X-ray if applicable
