Registered Nurse - Home Health - New Grads Welcome

About the role

Join an established New York healthcare organization providing skilled home health services across Brooklyn, Manhattan, and Queens. The organization supports patients across the lifespan, including well-baby, school-aged, postpartum, adult, and geriatric populations. As part of a broader healthcare organization, it offers the opportunity to build broad home health experience across different patient populations.

Responsibilities

• Complete approximately 5–6 skilled home health visits per day within an assigned geographic territory
• Perform admissions, initial assessments, OASIS documentation, and ongoing skilled nursing visits
• Provide care to a mix of pediatric, postpartum, adult, and geriatric patients
• Develop, implement, and document individualized plans of care in coordination with physicians and other clinical disciplines
• Educate patients and families on disease management, self-care, medications, and prevention strategies

Requirements

• Active New York State Registered Nurse license
• Reliable transportation and ability to travel throughout the assigned territory
• New graduates will be considered
• No ventilator or tracheostomy cases

Pay

• Salary: $100,000 - $120,000 per year
