Registered Nurse - Home Visiting Program

About the role

The North Central Health District’s Perinatal Health Home Visiting Program seeks a dynamic and passionate Registered Professional Nurse to provide essential home visiting services to expectant and new parents in rural areas of Georgia.

Responsibilities

• Provide professional nursing care in accordance with protocols, policies, and procedures.
• Focus on the prevention of illness, injury, or disability, and promote health and well-being.
• Care for populations with special needs during disasters/emergencies and biological/chemical protocols.
• Identify, assess, and provide care coordination services to eligible perinatal patients and qualifying infants.
• Coordinate program services with other service providers and community organizations.
• Screen and refer clients to other community service providers.
• Maintain accurate records of patient care, services, and responses to treatment.
• Perform physical assessments, take vital signs, fetal monitoring, and perform basic lab screenings.
• Provide breastfeeding education and counseling and refer clients to appropriate social and health services.
• Develop and sustain community partnerships.
• Deliver case management and nursing care according to public health protocols.
• Perform other duties as assigned, including inventory management, report preparation, clinic support activities, and coordination of community outreach initiatives.

Requirements

• A minimum of an Associate's degree in Nursing from an accredited college or university.
• Completion of a PHN course within one year of hire.
• Current Georgia Registered Professional Nurse license.
• CPR certification.
• A valid driver’s license and reliable transportation.

Qualifications

• Experience in home visiting services preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of public health principles and practices.

Skills

• Excellent verbal and written communication skills.
• Proficiency in Microsoft Office Suite.
• Ability to manage multiple tasks and prioritize effectively.
• Strong organizational and time management skills.

Schedule

This position is based at the North Central Health District’s Central Office in Macon, GA, and requires travel to provide home visits across various counties in Georgia. Employees are eligible to receive mileage compensation for work-related travel.
