Registered Nurse - Home Health - Sign-On Bonus

About the role

This is a part-time nursing position working 32 hours per week, primarily serving patients in the home care setting within the Southeast Region. The role involves evaluating patients, developing and implementing personalized care plans, and collaborating with an interprofessional team to ensure quality, evidence-based care. Nurses will assess patient needs, promote cultural competence, and adapt care based on condition changes, while providing education and support to patients and caregivers.

Responsibilities

• Assess patient preferences, values, and barriers to involvement in care.
• Develop, document, and communicate individualized care plans with defined goals.
• Promote respectful, equitable, and empathetic interactions with diverse populations.
• Evaluate patient condition changes, update care plans, and communicate with team and family.
• Ensure safe practices and adherence to standards in a safety-sensitive environment.

Qualifications

• Nursing Diploma/Associate's Degree in Nursing required; Bachelor's preferred.
• Valid RN license and driver's license required.
• Minimum of 2 years of experience; experience in home health or related fields is preferred.
• Strong critical thinking, communication, and cultural competence skills.
