Role Overview This role involves providing direct patient care as a registered nurse in a home care setting, emphasizing building meaningful patient relationships, assessing individual needs, developing personalized care plans, and collaborating with interdisciplinary teams to ensure quality outcomes. Key Responsibilities Evaluate patient preferences, barriers, and cultural needs to tailor care.Develop, implement, and document care plans with clear goals, ensuring effective communication across care teams.Promote respectful, equitable, and empathetic interactions with diverse populations.Monitor patient condition changes, update care plans, and communicate effectively with patients, families, and team members.Ensure evidence-based practice and safety in all care activities. Qualifications & Skills Nursing Diploma/Associate's degree in Nursing; Bachelor’s preferred.Valid RN license and driver's license.Minimum of 2 years nursing experience; 2-5 years preferred.Willingness to provide coverage across regions and participate in on-call duties.Strong critical thinking, communication, and cultural competency skills.
