Registered Nurse - Home Health - Sign-On Bonus

About the role

This is a part-time RN position involving comprehensive home care services within the Southeast Region, primarily serving patients in their homes. The role includes evaluating clients, developing and implementing individualized care plans, and collaborating with interdisciplinary teams to ensure quality, evidence-based patient outcomes. The nurse promotes cultural sensitivity, advocates for patients, and monitors changes in patient conditions to adjust care accordingly. Responsibilities also include providing education on self-care and coordinating care across various providers.

Responsibilities

• Assess patient preferences, barriers, and needs across emotional, spiritual, cultural, and health domains.
• Create, execute, and document personalized care plans in collaboration with patients, families, and team members.
• Deliver respectful, equitable, and compassionate care, integrating data and critical thinking for timely interventions.
• Evaluate patient status changes, communicate updates, and ensure adherence to best practices and safety standards.

Qualifications & Skills

• Minimum of a Nursing Diploma/Associate's degree and RN licensure.
• At least 2 years of nursing experience; preferred Bachelor's degree in Nursing or Home Health with 2–5 years of experience.
• Valid Driver's License required.

Additional Requirements

• Willingness to provide coverage across all regions during on-call rotations.
• Registration with the Family Care Safety Registry is mandatory.
