Registered Nurse - Home Health & Hospice
Position Overview
Seeking a skilled and experienced Registered Nurse to provide high-quality home health care to patients in Howard and Carroll County. This field-based role focuses on delivering individualized care, managing complex clinical needs, and supporting patients in a home setting to improve health outcomes and quality of life.
Schedule & Work Details
- Location: Howard and Carroll County coverage area
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- We guarantee 32 hours per week
- On-call/Weekend Requirement: On-call required
- Mileage Reimbursement: Yes, between patient visits (first to last patient)
- Pay Rate: $51 to $53 per hour
- Dress Code: Navy blue scrubs
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and plan of care
- Perform patient assessments, identify changes in condition, and adjust care plans as needed
- Administer medications, treatments, and specialized procedures
- Manage complex clinical tasks including central line care, wound care, wound vacs, and catheter care (foley and suprapubic)
- Perform venipuncture and specimen collection
- Document patient care accurately and timely in accordance with agency and regulatory standards
- Collaborate with physicians and interdisciplinary care team to coordinate patient care
- Educate patients and caregivers on disease management, medications, and home care techniques
Required Qualifications
- Active Registered Nurse (RN) license in the appropriate state
- Prior home health experience required
- Strong clinical assessment and documentation skills
- Ability to work independently and manage a field-based schedule
Preferred Qualifications
- Experience with pleurex drains
- Experience with central line care
- Proficiency in wound care and wound vac management
- Experience with catheter insertion (foley and suprapubic)
- Strong venipuncture skills
- Excellent time management and communication skills
- Experience with Homecare Homebase (HCHB) is a plus
