Role Overview

We are seeking a registered nurse to provide comprehensive home care services, including skilled nursing, hospice, rehabilitation, and specialty programs. The role involves evaluating patients, developing and implementing personalized care plans, and collaborating with an interprofessional team to ensure quality, patient-centered care. The nurse will assess patient needs, promote cultural sensitivity, and communicate effectively with patients, families, and team members.

Requirements

• Valid RN license
• Nursing diploma or associate's degree
• Minimum of two years of experience
• Bachelor’s degree preferred, with 2-5 years of experience in home health
• Valid driver’s license
