Registered Nurse - Home Health

Role Overview

This position involves providing individualized home health nursing care, supporting underserved communities, and building meaningful patient relationships within a team environment. The RN evaluates patient needs, develops and implements personalized care plans, and collaborates with interprofessional teams to ensure quality, evidence-based care.

Responsibilities

• Assess and address patient needs, values, and barriers to care
• Develop, implement, and document care plans with team and patient input
• Provide culturally sensitive, patient-centered care promoting respect and empathy
• Monitor and evaluate patient progress, communicate updates, and adjust plans as needed
• Collaborate effectively within interdisciplinary teams to ensure continuity of care
• Maintain safety and adhere to regulatory standards

Qualifications

• RN license
• Valid driver’s license
• Nursing diploma or associate’s degree (bachelor’s preferred)
• Experience: less than 2 years (required), 2-5 years (preferred)
