Registered Nurse – Home Health

About the role

We are seeking a Registered Nurse (RN) for Home Health at HopeWest Hospice. This is a 13-week contract position offering competitive pay and the opportunity to provide compassionate, community-based care to elderly participants through the PACE (Program of All-Inclusive Care for the Elderly) program.

Responsibilities

• Deliver high-quality nursing care in a home health setting while working independently and coordinating with an interdisciplinary team.
• Work in an acute care and home health setting, providing care to elderly participants through the PACE program.

Requirements

• Active RN License (CO or Compact)
• Minimum 2 years of recent hospital or acute care experience
• Home Health and/or LTC Experience (preferred)
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication and organizational skills
• Experience with EHR systems and Microsoft Office
