About the role
We are seeking a Registered Nurse (RN) for Home Health at HopeWest Hospice. This is a 13-week contract position offering competitive pay and the opportunity to provide compassionate, community-based care to elderly participants through the PACE (Program of All-Inclusive Care for the Elderly) program.
Responsibilities
- Deliver high-quality nursing care in a home health setting while working independently and coordinating with an interdisciplinary team.
- Work in an acute care and home health setting, providing care to elderly participants through the PACE program.
Requirements
- Active RN License (CO or Compact)
- Minimum 2 years of recent hospital or acute care experience
- Home Health and/or LTC Experience (preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication and organizational skills
- Experience with EHR systems and Microsoft Office
