Registered Nurse (Home Health)

Home Care Registered Nurse provides person-centered nursing care to clients in their homes. Key duties include performing comprehensive assessments, developing and updating individualized care plans, administering medications and treatments, and monitoring both chronic and acute conditions. The RN coordinates with physicians and interdisciplinary teams—including behavioral health specialists, therapists, and social workers—to deliver evidence-based, outcomes-focused care. The role also includes educating patients and families on care needs and self-management. Additional responsibilities include accurate documentation in an electronic medical record (EMR), promoting client safety and infection control practices, and supporting quality improvement efforts while collaborating with public-sector partners.
