Registered Nurse (Home Health)

About the role

Provide direct patient care in clients' homes, collaborating with physicians and healthcare team members to develop and implement personalized care plans.

Responsibilities

• Initiate, revise, and supervise care plans and oversee home health aides and nurses.
• Conduct comprehensive assessments, ensure documentation accuracy, and maintain compliance with regulations.
• Communicate effectively with patients, families, and healthcare providers to promote quality care.
• Participate in team activities, orientation, performance improvement, and survey readiness.
• Ensure safety and adaptability in diverse home environments.

Requirements

• Current, unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Qualifications

• Preferred: CPR certification.
• Preferred: 1+ years of home health experience.
• Preferred: Strong communication and organizational skills.
• Preferred: Ability to work independently.
