Registered Nurse Home Care- Per Diem (Middlesex/Somerset/Union counties)

Responsibilities

• Provide professional nursing care to patients in their home.
• Achieve compliance with physician orders and maintain visit frequency.
• Document care plans and interventions accurately.
• Care holistically for patients and their significant others.
• Ensure safe patient care and complete assigned treatments and procedures.
• Collaborate with interdisciplinary teams and document activities.
• Identify appropriate payors and remain compliant with authorizations.
• Meet productivity and documentation timelines.
• Supervise para-professional home health aides and document supervisory visits.
• Maintain awareness of new practices and incorporate them into nursing care.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• At least six (6) months RN experience or at least 1 year of LPN experience in home care or hospice.
• BLS certification.
• NJ State Professional Registered Nurse License.
• IV certification preferred.
