Responsibilities
- Assess patient needs and develop care plans
- Coordinate with healthcare providers to ensure continuity of care
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients and families to provide support and education
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in Nursing or related field
- Current RN license in North Carolina
- At least 2 years of home health care experience
- Excellent communication and interpersonal skills
Qualifications

- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
