Registered Nurse Case Manager (Homecare)

What We Offer

Responsible for coordinating home care services which includes establishing and maintaining the patient’s plan of care. Responsible for the quality, customer service and financial management of assigned case load.

What We're Looking For

• Educational Requirements: High School Diploma or GED, required. 2 Year / Associate Degree Associates Degree in Nursing from approved school, required. 4 Year / Bachelors Degree BSN, preferred.
• Experience Requirements: 1 year of experience of recent acute care, home health or related experience, required.
• Licensure/Certification Requirements: Current RN licensure in state of employment, required. BLS, required. Valid Driver’s License, required.
• Additional Skills: Hours of Work: Flexible hours to meet educational, departmental and institution needs.
