Registered Nurse, Home Care

About the role

The position to which you are applying is represented by a collective bargaining unit, Massachusetts Nurses Association. This role involves leading with both skill and heart in patients' homes, not hospital hallways. Responsibilities

• Provide quality, clinical services to patients in various settings in the community.
• Deliver skilled nursing care in the form of assessment, teaching, treatment, documentation, and care coordination.
• Collaborate closely with LPNs, therapists, and social workers to oversee the full plan of care.
• Complete 5-6 visits per day, with time for documentation, chart review, and coordination built into your schedule.

Requirements

• Graduate of an approved School of Nursing.
• Current Massachusetts license as Registered Nurse.
• Minimum of one year previous nursing experience in acute care setting.
• Prior home care experience preferred.
• Ability to work with various computer software and knowledge of hand-held patient care computer devices.
• Strong interpersonal and customer service skills, and ability to work with a diverse population.
• Effective communication in writing, verbally, and electronically.
• Organized in time management and comfortable navigating a dynamic home-based environment.
• Confident in clinical decision-making and adaptable to unpredictable circumstances.
• Ability to work independently, be self-directed, and adapt to changing situations.
• Experience working as a member of an interdisciplinary team of health care providers.
