Registered Nurse, Home Care
About the role

This role involves leading with both skill and heart in patients' homes, not hospital hallways. Responsibilities
- Provide quality, clinical services to patients in various settings in the community.
- Deliver skilled nursing care in the form of assessment, teaching, treatment, documentation, and care coordination.
- Collaborate closely with LPNs, therapists, and social workers to oversee the full plan of care.
- Complete 5-6 visits per day, with time for documentation, chart review, and coordination built into your schedule.
Requirements
- Graduate of an approved School of Nursing.
- Current Massachusetts license as Registered Nurse.
- Minimum of one year previous nursing experience in acute care setting.
- Prior home care experience preferred.
- Ability to work with various computer software and knowledge of hand-held patient care computer devices.
- Strong interpersonal and customer service skills, and ability to work with a diverse population.
- Effective communication in writing, verbally, and electronically.
- Organized in time management and comfortable navigating a dynamic home-based environment.
- Confident in clinical decision-making and adaptable to unpredictable circumstances.
- Ability to work independently, be self-directed, and adapt to changing situations.
- Experience working as a member of an interdisciplinary team of health care providers.
