Registered Nurse, Home Care

About the Role

Mass General Brigham Home Care is seeking a full-time Registered Nurse to join the "Tanner City" team. While the majority of your time will be working in Topsfield and Boxford, the entire team also supports Danvers, Hathorne, Lynnfield, Middleton, Peabody, N. Andover, N. Reading, Hamilton, S. Hamilton, and Wenham. We will secure city parking passes for new team members to make the commute as convenient and stress-free as possible.

Responsibilities

• Serve as case manager, overseeing the full plan of care while collaborating closely with LPNs, therapists, and social workers
• Complete 5-6 visits per day, Monday through Friday, with time for documentation, chart review, and coordination built into your schedule
• Provide skilled nursing care in the form of assessment, teaching, treatment, documentation, and care coordination
• Perform admissions process and case management responsibilities in accordance with MGB Healthcare at Home policies and standards of practice
• Utilize thorough and timely electronic documentation and interdisciplinary communications
• Deliver high-quality customer service and maintain financial awareness
• Travel within MGB Healthcare at Home geographic area

Qualifications

• Graduate of an approved School of Nursing
• Current Massachusetts license as Registered Nurse
• Minimum of one-year previous nursing experience in acute care setting
• Prior home care experience preferred
• Ability to work with various computer software required; knowledge of hand-held patient care computer devices preferred
• Strong interpersonal and customer service skills with ability to work with a diverse population
• Ability to communicate effectively in writing, verbally, and electronically
• Ability to work as a member of an interdisciplinary team of health care providers
• Ability to organize and prioritize work, and adapt to changing situations
• Ability to work independently, be self-directed, and adapt to unpredictable circumstances

Pay

$40.09 - $71.12/hourly
