Role Overview

Key Responsibilities

• Assess patient status through interviews, observations, and physical assessments.
• Aid in developing individualized care plans collaboratively with patients, families, and the healthcare team.
• Implement interventions to achieve patient outcomes and evaluate progress continually.
• Document patient care accurately, including charting and event reports.
• Maintain professional accountability, seek resources, and pursue self-development.
• Operate and troubleshoot medical equipment, staying current with evidence-based practices.
• Share clinical knowledge, mentor team members, and adapt teaching to patient/family needs.
• Participate in interdisciplinary care, data collection, and process improvement efforts.
• Ensure care aligns with standards, safety goals, and quality initiatives.

Qualifications & Skills

• Licensed Practical Nurse (LPN) license and Basic Life Support (BLS) certification.
• Strong assessment, clinical, and documentation skills.
• Knowledge of evidence-based nursing practices and patient safety standards.
• Ability to collaborate effectively within a healthcare team.
• Commitment to ongoing learning and professional development.
