Responsibilities

• Provide compassionate, holistic care including assessments, care planning, medication administration, vaccines, and patient education
• Collaborate with medical, dental, behavioral health, and wellness teams to support integrated, whole-person care and promote health equity
• Coordinate follow-up care and manage chronic conditions
• Maintain accurate electronic health record (EHR) documentation
• Uphold safety and quality standards in patient care
• Model a mission-driven, collaborative culture

Qualifications & Skills

• Current RN licensure
• Strong clinical assessment and care coordination skills
• Ability to work effectively within a multidisciplinary team
• Excellent communication and patient education abilities
• Commitment to community health and health equity
