About the role

The RN Case Manager - Home Hospice plays a vital role in delivering compassionate, patient-centered end-of-life care in patients' homes.

Responsibilities

• Conduct comprehensive assessments of physical, emotional, and psychosocial needs
• Develop and update individualized care plans
• Coordinate with interdisciplinary teams including physicians, social workers, and chaplains
• Educate and support patients and families on disease progression and symptom management
• Monitor patient status and adjust care as needed
• Ensure compliance with hospice regulations and documentation standards
• Facilitate admissions, transfers, and discharges
• Advocate for patient preferences respecting cultural and spiritual values

Requirements

• Valid RN license
• At least two years of clinical experience (preferably in hospice or palliative care)
• Proficiency in electronic medical records
