Role: Registered Nurse providing in-home care across Western Colorado. Responsibilities Conduct comprehensive patient assessments.Develop and update personalized care plans.Administer medications and treatments safely.Manage wound care, post-surgical, and chronic conditions.Collaborate with interdisciplinary teams to ensure quality care.Educate patients and families on health management.Document care activities accurately and thoroughly.Triage patient needs and coordinate necessary services.Support safe care transitions and promote patient independence. Qualifications & Skills Current RN license.Strong assessment and care planning skills.Experience with wound, post-surgical, and chronic disease management.Excellent communication and patient education abilities.Ability to collaborate effectively within a team.Detail-oriented with accurate documentation skills.
