Registered Nurse Emergency Department Case Manager RN

Position Purpose

The Emergency Department Registered Nurse Case Manager at Saint Francis Hospital plays a critical role in coordinating patient care in a fast-paced setting. This role focuses on early identification of patient needs, care coordination, utilization management, and safe transitions of care to ensure high-quality, cost-effective outcomes while supporting patient flow and throughput from the Emergency Department.

What You Will Do

• Collaborating with ED providers and care teams to assess patient needs upon presentation
• Identifying appropriate levels of care and facilitating admissions, transfers, or safe discharges
• Coinciding care across inpatient, observation, and post-acute settings
• Supporting patient flow, throughput, and length-of-stay initiatives
• Conducting clinical, psychosocial, and discharge planning assessments
• Applying nationally recognized medical necessity and utilization criteria
• Ensuring compliance with regulatory, accreditation, and payer requirements
• Educating patients and families on care plans, resources, and next steps
• Advocating for patients while balancing quality, safety, and resource utilization

Required Minimum Qualifications

• Graduation from an accredited nursing school
• Valid RN License in Connecticut
• Minimum of three (3) years of acute care RN experience required
• Emergency Department experience strongly preferred
• Demonstrated knowledge of utilization management, level-of-care determination, and care coordination
• Working understanding of CMS/HCFA, OBRA, State of Connecticut regulations, and Joint Commission (JCAHO) standards
• BLS certification required
• Certification in Case Management (CCM or ACM) preferred or obtained within a designated timeframe
