Registered Nurse RN Case Manager

The role requires the teammate to support both ESKD and CKD patients throughout California, Oregon, and Washington, with flexibility to reside in a surrounding city.

• Supports markets across California, Oregon, and Washington
• Requires travel throughout the assigned territory

Responsibilities

• Holds a valid RN license in California, Oregon, and Washington, or willing to obtain licensure in any one of these states
• Supports patients in both ESKD and CKD markets
• Works with nephrology partners to meet patient needs

Requirements

• Valid RN license in California, Oregon, and Washington
• Flexible schedule to accommodate patient needs and partner availability

Qualifications

• Experience in nephrology care
• Strong communication and teamwork skills
