Responsibilities
- Conduct home visits, assess patient environments
- Train patients and care partners
- Document care in electronic records
- Maintain infection control
- Manage emergency procedures
- Cook supplies
- Track patient outcomes
- Perform lab tests and medication reviews
- For home hemodialysis, coordinate home evaluations and water sampling
- Perform on-call nursing services as needed
- Maintain compliance with regulations
Qualifications
- Current RN license
- Minimum 12 months of clinical experience, with specific home dialysis experience preferred
- CPR certification
- Good color vision
- Computer skills
- Successful completion of orientation
