Responsibilities

  • Determines the need for immediate physician attention and executes medical orders.
  • Coordinates nursing care with routine facility activities and individual treatment plans.
  • Adheres to applicable regulations and reviews resident progress as needed.
  • Conducts pre-admission evaluations and develops care plans.
  • Periodically reassesses the type, extent, and quality of services and programming.
  • Develops discharge plans and refers residents to appropriate community resources.
  • Provides in-service training on personal hygiene, medication administration, family life, and sex education.
  • Controls communicable diseases and infections, implementing protective and preventive measures.
  • Develops and implements treatment plans in collaboration with the interdisciplinary team.
  • Records nursing observations, actions, medications, treatments, reactions, tests, intake and output, and vital signs in medical records.
  • Supervises medication administration and ensures proper disposal of outdated and discontinued drugs.
  • Works on-call and varies shifts as needed.
  • Collaborates with consulting and contracted medical services to ensure recommended treatments are carried out and documented.
  • Maintains inventory standards for medicines, solutions, supplies, and equipment to meet nursing care needs.
  • Trains direct support professional staff in nursing techniques and the use of certain nursing equipment.
  • Serves as a liaison, interpreter, or accompaniment to individuals with outside providers of health services.
  • Ensures compliance with agency policies and guidelines.

Qualifications

  • Licensed by NYS Dept. of Education.
  • Experience in OPWDD Certified homes, community residences, and nursing homes.
  • First Aid and CPR certifications preferred.
  • Respects and considers the resident being served.
  • Valid Driver’s license preferred.
  • On-call via phone and Lotus Notes access 24/7 days per week.
