Registered Nurse Coord (RN) - Transfer Center

About the Role

Coordinate Transfer Center transfers from the initial call through patient discharge and follow-up, providing the highest quality service to physicians, nurses, patients, and other medical professionals.

Responsibilities

• Respond to all referrals to the Transfer Center program in a timely manner.
• Contact the receiving nurse to outline the patient’s condition, medical treatment plan, and physical orders to prepare for the patient’s arrival.
• Assist with development and implementation of educational programs for nurses (e.g., Lunch & Learns, ACNS).
• Facilitate physician-to-physician communication.
• Communicate with sending facility to ensure follow-up with the physician is accomplished.
• Assist sending facility with use of air or ground transport of patient.
• Telephone receiving facility Admitting Department to admit patient.

Requirements

• Associate’s Degree in Nursing, or equivalent, from an accredited program.
• 4 years of clinical experience in an acute care setting.
• Current Arizona, or other compact state, Registered Nurse license.
• Basic Life Support (BLS) certification.

Preferred Qualifications

• Bachelor of Science in Nursing.
• RN experience in an ER/Trauma, Neurosciences, and/or Cardiovascular setting.
• Knowledge of, and ability to apply, InterQual criteria.
• Advanced Cardiovascular Life Support (ACLS) certification.
• Applicable professional certification (e.g., CCRN, CEN, CNRN).

• Ability to interact and communicate in English, both orally and written, with all levels of staff, physicians, patients, and families in an effective manner, exhibiting tact, enthusiasm, and patience.
• Sensitivity to cultural and bilingual issues.
